Living Choice Eligibility
To be eligible for the Living Choice Demonstration, a person must be:
- 19 years or older
- Have a disability or long term illness
- Meets nursing facility level of care
- Medicaid eligible (See Financial Guidelines For Eligibility)
- A total of 90 days consecutive stay in a long term care institutional setting (i.e. nursing home, intermediate care facility, skilled nursing, hospital, PRTF, FQHC)
- At least 1 day must be paid by Medicaid.
- Must not be a resident of a NF in lieu of incarceration.

Medically Fragile Eligibility
To be eligible for the Medically Fragile Waiver Program, a person must be:
- Be at least 19 years of age
- Meet hospital and/or skilled nursing level of care
- Medicaid eligible (See Financial Guidelines For Eligibility)
- Must be living at home or a residential setting
There are income and asset limits to eligibility for long-term care services. LTCWO pays for care for those with middle incomes only after they have exhausted their own financial resources. You may visit or call your local county office of the Oklahoma Department of Human Services to apply.
