340:105-12-32. Required United States Department of Labor (DOL) forms

(a) Sub-grantees use DOL forms for the Senior Community Service Employment Program to include the:

(1) Participant Handbook Acknowledgment;

(2) Family Size Verification;

(3) Family Income Worksheet;

(3) Host Agency Agreement;

(4) Host Agency Orientation; and

(5) Job Search Activity Documentation.

(b) The DOL Participant Handbook or completion of forms do not constitute a contract or a guarantee to obtain work.
