Provides vaccine assistance for eligible individuals, including the homebound and their family caregivers. Assistance can include but is not limited to: 

  • Scheduling an appointment
  • Identifying transportation to a vaccine site
  • Review vaccine record and suggested vaccines
  • Deciphering misinformation
  • In-home vaccine for those homebound due to a medical condition

Family caregivers are encouraged to use this resource for themselves and their loved ones.

This toll-free statewide telephone number links individuals with special needs across the lifespan and their family caregivers to information and assistance at the local level. Calls are routed to the appropriate Area Agencies on Aging or Sooner SUCCESS specialists based on the prompts chosen.

Assists functionally impaired adults, generally age 60 and over, with community-based care that allows them to remain in their homes.

Provides services that allow Medicaid-eligible persons who meet nursing facility level of care to remain at home, or in the residential setting of their choosing, while receiving necessary care.

Investigates complaints on behalf of long-term care residents with the goal of improving quality of life and quality of care. The Ombudsman program serves residents in Oklahoma’s long-term care facilities including nursing homes, assisted living and similar adult care homes.

Assists individuals that meet the program’s income and resource eligibility requirements by lowering their Medicare prescription drug costs, deductibles, and premiums and provides information on preventive health services.

Offers free services to Oklahomans 60 years or older to help maintain their dignity and independence. The Area Agencies on Aging (AAA) network reviews the person’s needs and offers

services to meet those needs. The services are offered to assist older Oklahomans to avoid being placed in a nursing facility and to live independently in their own homes.
