This School Food Authority (SFA) operates the National School Lunch Program and the School Breakfast Program.

The following areas were corrected prior to the On-Site Portion of this Administrative Review (AR):

  • Local Wellness Policy: The local wellness policy provided during the Off-Site portion of this AR did not contain goals for other school based activities that promote student wellness [7 CFR Part 210.31(c) (1)].  Prior to the On-Site portion of this AR the School Food Authority (SFA) provided a copy of an updated local wellness policy and included goals for other school based activities that promote student wellness.

    During the Off-Site portion of this AR Unity Point Counseling and Resource Center, Inc. (UPCRC) did not provide a copy of their most recent triennial wellness assessment report documenting the most recent local wellness policy assessment and update [7 CFR Part 210.31 (e) (2) and USDA Memo SP24-2017].  Prior to the completion of the On-Site portion of this AR UPCRC provided their most recent triennial wellness assessment report.

     
  • Meal Patterns:  Insufficient serving sizes were discovered for the breakfast and lunch menus assessed (07/13/20 through 07/19/20) [7 CFR Part 210.10 (c) and 7 CFR Part 220.8 (c)].  Corrective action was taken prior to the on-site portion of this AR for all insufficient serving sizes discovered for the breakfast menus during the week reviewed.  The SFA took corrective action prior to the completion of the on-site portion of this AR for the following insufficient serving sizes discovered for lunch menus during the week assessed: the insufficient bread/grain serving on 07/16/20 and the insufficient bread/grain serving on 7/17/20.

  • The SFA served non-creditable food items as part of the meal component:During the menu week assessment of breakfast and lunch menus (for the week of 07/13/20 through 07/19/20) it was determined UPCRC served non-creditable items (7 CFR Part 210 Appendix C). The following non-creditable items were served with lunches: the chicken used with the menu on 07/13/20, and the corndog served on 07/18/20.  Prior to the completion of the on-site the SFA took corrective action replace both items with creditable items.

Corrective action for the above was made prior to the On-Site portion of this AR.  No further action is required for the above.  

Findings and Recommendations Identified During the Off-Site Portion of the Review Conducted August 18, 2020:

 
Meal Patterns:A review of 07/13/20 through 07/19/20 indicated some insufficient portion sizes at lunch. 

 
The lunch meal pattern 7 CFR 210.10 (c) requires the following minimum serving sizes; 2 oz. by weight meat/meat alternate daily and 14-17 oz. per week, 2oz. by weight bread/grain daily and 14-17 oz. per week, 1 cup fruit daily and 7 cups per week, 1 cup vegetables daily (dark green subgroup ½ cup/week, red/orange subgroup 1 ¼ cup/week, legumes subgroup ½ cup/per week, starchy subgroup ½ cup/week, and other subgroup ¾ cup/week) 7 cups/week, and 1 cup fluid milk daily, 7 cups weekly, offered as a choice between 1% and fat free unflavored or flavored milk.


Insufficient quantities

  • The menu served on 07/14/20, had an insufficient daily serving size for the bread/grain component, the bun served only credits at 1.75 oz. [7 CFR Part 210.10 (c)].  The minimum daily serving size for the bread/grain component is 2 oz. [7 CFR Part 210.10 (c)].
  • The menu served on 07/19/20 had an insufficient daily serving size for the meat/meat alternative component, the chicken served only credited 1 oz. toward this component [7 CFR Part 210.10 (c)]. The minimum daily serving size for the meat/meat alternative is 2 oz. [7 CFR Part 210.10 (c)].
  • Insufficient weekly serving for the meat/meat alternative component, only 9.75 oz. could be credited to this weekly serving [7 CFR Part 210.10 (c)].  The minimum required weekly serving size for the meat/meat alternative component is 14oz. [7 CFR Part 210.10 (c)].  This insufficient serving size was due to one daily insufficient serving size and two menu days where the items served to meet the meat/meat alternative component daily serving size were non-creditable items.  The corrective actions taken prior to the on-site brought the weekly serving to 13.75 oz., which is 0.25 oz. below the minimum weekly serving.  Once the above insufficient daily meat/meat alternative component serving size is corrected this will bring the weekly serving into compliance.

The above insufficient serving sizes for the breakfast and lunch menus served during the week of 07/13/20 through 07/19/20 were determined to be non-systemic in nature.

The State Agency (SA) is exercising its discretion to not apply fiscal action for the above insufficient quantities as allowed for under 7 CFR Part 210.18 (g) (2). 

It is very important for your staff to carefully review planned menus, product labels and recipes to ensure, the meal pattern requirements are met for every meal claimed for reimbursement. It is critical that your staff document everything that is served as part of a reimbursable meal.



To demonstrate compliance with Meal Patterns:

  • Review your menus for compliance with the meal patterns for grades 9-12.
  • Bring all insufficient serving sizes and missing components in compliance with the meal patterns for grades 9-12.
  • Submit your plan for corrective action to demonstrate understanding and compliance.

The following areas were corrected during the Procurement Review (PR):

  • Procurement: The procurement plan provided during the AR off-site did not include the requirement that a cost or price analysis be conducted with every procurement in excess of the small purchase threshold [2 CFR Part 200.323 (a)].  Prior to the completion of the on-site portion of the AR the SFA provided a procurement plan requiring a cost or price analysis for all procurements in excess of the small purchase threshold.

Corrective action for the above was made during the PR.  No further action is required for the above

There were no additional findings for the procurement review.
