Program Information
Long Term Care through Medicaid is a needs-based healthcare program that covers the cost of long-term care for seniors and disabled individuals who meet the state’s eligibility requirements.

Financial Eligibility Checklist for Long Term Medicaid
Individuals applying for Long-term Care Medicaid must provide documentation verifying any and all income and assets. If married, then all the couple’s resources and assets must be provided regardless of name and ownership.
1. Social Security, Medicare, and health care insurance cards. Proof of citizenship (i.e., birth certificate, naturalization certificate) and copy of photo identification (i.e., driver’s license). Permanent resident card if applicable.
2. Copies of policies and/or documents related to life insurance, burial policies, plots, and/or trust funds, current cash value in life insurance policies owned solely or jointly by the applicant.
3. Proof of income, both gross and net, showing deductions (i.e., civil service pensions, military pensions, company pensions, social security, oil and gas royalties, rent-al property income, interest, and dividend income, etc.). Proof of spousal income for Nursing Home applicants.
4. All current bank statements showing the amount of monies available to the applicant. Current paperwork verifying stocks, bonds, annuities, IRA accounts, certificates of deposit, safety deposit box contents, and mineral rights showing fair market value and income received within the past 12 months.
5. Legal descriptions and copies of deeds for home property, and other owned real estate, which includes mineral rights.
6. Titles to vehicles, boats, motorcycles, recreational vehicles, etc. (One vehicle is exempted)
7. Documentation is showing any transfer of property, money, or assets in last 60 months (5 years) belonging to applicant.
8. Proof of medical expenses such as monthly health insurance premiums, prescription costs, medical supplies, and payments to medical suppliers.

Options on How to Apply:
- Call (405) 522-5050. Press 1 for English or 2 for Spanish, then press 1, and finally 4 to receive assistance from a Family Services Specialist.
- Visit a local Oklahoma Human Services Center to speak with a representative in a FaceTime platform using OKDHS provided tablet.
- For ADvantage or State Plan Personal Care services, please apply online – click here, Application for In-Home Assistance (mediware.com).
- For Nursing Home, the nursing home of your choice will submit an application to Oklahoma Human Services.
