Sexual Assault Kit Tracking System Registration

Medical facilities that perform sexual assault forensic exams, law enforcement agencies, forensic labs, and other entities that have custody of sexual assault kits are required by law to use the Sexual Assault Kit Tracking system. This registration form only applies to medical, law enforcement, and forensic lab personnel that will be sending and receiving sexual assault evidence collection kits.
