Birth Certificates
Plan ahead! Ordering birth certificate copies online — ideally before August — is the fastest way to receive records before school starts.
Shot Records
Shot records can be requested through the Oklahoma State Immunization Information System or from your child’s health care provider.
Immunizations
If your child needs an immunization, reach out to your county health department or health care provider to schedule an appointment. Please review the childhood immunizations guide for more information. If your child is exempt from immunizations for medical, religious or personal reasons, you can complete an exemption certificate.
Well-Child Exams
Contact your health care provider or county health department to schedule a well-child exam before the back-to-school rush.
- An official copy of a birth certificate is typically required for students enrolling for the first time or enrolling at a new school.
- Order online or by phone at (877) 817-7364. Requests from a parent named on the birth record are typically issued within two business days and arrive in the mail within a week. Will call pickup is also available.
- Birth certificate with no changes: $15
- One copy will be issued if the birth record is found. The fee is non-refundable if the birth record is not found. Additional copies are $15 each.
- Change or amendment to a birth record: $40
- Online / phone processing fee: $5 for Oklahoma residents (varies for non-residents)
- Online / phone expedite fee: $20 (if applicable)
- Records requiring a change or amendment are not eligible for expedited services
- As soon as possible. If you wait until August, you’ll likely experience a delay due to a high volume of order requests.
- There are delivery and will call options. The delivery option for birth records requested online by a named parent are typically processed within two business days and delivered by standard mail within a week. This option avoids long lines, parking costs and your time. Orders can be sent by express mail for an additional fee. The will call option requires a scheduled pick-up time at one of the three Vital Records offices in Oklahoma City, Tulsa or McAlester.
- Requests should be placed by a parent named on the birth certificate. If the order is placed by a non-named parent, additional documentation is required.
- Requests should be completed in full without abbreviations or blanks.
- Don’t forget to include parental middle name and mother’s maiden name (last name prior to first marriage).
- Double check the request before submitting to make sure the information is correct.
- Call (866) 203-2777 to check the status of orders made online or by phone. You can also check your email. Vital Records will contact you with updates.
- Any requests for amendments or changes to birth certificates can take up to 8-12 weeks to process. We recommend taking to school the birth certificate as is with documentation demonstrating the correct spelling or adjustments. The best time to request an amendment or change is September – December.
- Second parent requests can take up to 8-12 weeks to process. We recommend taking to school the birth certificate as is and the completed Acknowledgement of Paternity or a parentage order from the court that officially recognizes the second parent.

- The best time to request a second parent addition is September – December.
- When you submit a request, upload a copy of your court appointed guardianship order.
- The search query is case sensitive. You might also try testing a middle initial vs. a middle name. Enter the information exactly as it appears on your child’s birth certificate. If your child’s information still doesn’t appear, please first email OSIISHelp@health.ok.gov. If records still cannot be found, you can call your county health department or contact your health care provider.
- Immunization recommendations vary by age. Please review the childhood immunizations guide for more information.
- All children aged two months and older must present an immunization record or file an exemption to attend public school. Oklahoma law allows exemptions for medical, religious and personal reasons. Exemption certificates are available in English and Spanish. Please note: exemption certificates that are not properly completed or have missing information will not be approved and will be returned to the school for the parent/guardian to correct or complete.
- Well-child exams provide an opportunity to talk with your child’s health care provider about developmental and behavioral milestones. This might include discussing how your child is growing and developing, their health history, growth measurements, healthy sleep and nutrition habits, and more.
- Although a well-child checkup can include the exam required for a sports physical, it covers much more. In-depth information like your child’s health history, growth measurements, blood pressure, any required lab tests, as well as development and behavior screenings are all included in a well-child exam.
- Yes. Well-child exams are recommended, not required. The Oklahoma State Department of Health suggests you complete well-child exams to ensure your child is in optimum health to perform and succeed in school.
