Effective November 1, 2006, except as otherwise provided by the Elevator Safety Act, every elevator in this state shall be subject to the provisions as required by this act. Within six (6) months of November 1, 2006, the owner or lessee of every elevator already in service or put into service by November 1, 2006, shall register the elevator with the Department of Labor, giving the type, rated load and speed, name of manufacturer, location of the elevator, and purpose for which used, as well as such other information as the Commissioner of Labor may require. Elevators newly constructed or installed on or after November 1, 2006, shall be registered and inspected before being put into service.

  • Inspection frequencies vary. Please refer to OAC 380:70-3-5.
  • H. Elevators, escalators, and other such devices within the definitions of the Elevator Safety Act shall receive an inspection for the purpose of obtaining a certificate of operation: 1. Two-floor to four-floor elevator units, not to exceed two (2) years; 2. Any wire-rope elevator, regardless of floors, annually; 3. Escalators and moving walkways, annually; 4. Wheelchair lifts, triennially; 5. Temporary elevators shall be inspected at each erection and every ninety (90) days or as the code requires; and 6. Any elevator or other such device subject to the provisions of the Elevator Safety Act located in a structure whose occupants are mobility restricted, such as hospitals, nursing homes, and residential care facilities, shall be inspected annually.

After inspection is processed you will be invoiced out of our office, please return a copy of the invoice with the payment.

At this time online payment is not an option.

If an invoice was issued, you will receive your certificate once the fee has been paid. If the fee has been paid, please provide us with the OK# and we will verify if we have received payment.

No, please refer to OAC 380:70-11-8 380:70-11-8. Major repairs and alterations: Permits (e) When an elevator or conveyance with a valid Certificate of Operation undergoes an alteration or major repair, the Certificate of Operation shall become invalid until such time as the conveyance undergoes an inspection by the Commissioner, Chief Inspector, or Deputy Inspector. Reports of such alteration or major repair shall be provided to the Department prior to the inspection and shall be on forms acceptable to the Department.

Certificates of operation

(d) The required Certificates of Operation shall be posted conspicuously as follows:

 (1) inside elevator cars or in the main lobby (landing) adjacent to elevator call station, or 

 (2) for escalators, at the base of the balustrade at the lower level of the escalator. 

(e) The Certificate of Operation shall be conspicuously displayed in, upon, or adjacent to the conveyances for viewing by the general public.

Please refer to OAC 380: 70-3-1 59 O.S. § 3023 Elevator Inspection Bureau – Duties of the Chief Elevator Inspector – Rules H. Elevators, escalators, and other such devices within the definitions of the Elevator Safety Act shall receive an inspection for the purpose of obtaining a certificate of operation: 380:70-3-1. Responsibilities (3) The owner, his duly appointed agent, or the lessee shall be responsible for the safe operation and proper maintenance of elevators, escalators, moving walkways, wheelchair lifts and conveyances after the installation has been approved and a Certificate of Operation has been issued. The owner his duly appointed agent, or the lessee shall be responsible for conducting all periodic maintenance and/or testing as required by these rules. (4) The owner, his duly appointed agent, or the lessee shall also be responsible for having conveyances subject to the provisions of this Act inspected as required by the Act. Failure to do so will subject the owner, his duly appointed agent, or lessee to penalties and/or administrative fines as provided by this Act and these rules.

Things to keep in mind before deciding what type of service contract

a. age of elevator

b. having to repair frequently (breakdown)

c. number of trips on a daily bases (used often)

d. what type of clientele (nurse home, hospital, medial facility, retirement living etc..)

e. how many floors does it serve

All Elevators:

1. Contact the Department have needed information readily:

a. Decommissioning licensed contractor

b. Start date

c. Location

d. Identify elevators decommissioning (Ok#, car #, freight etc...)

e. Follow-up inspection shall be schedule upon completion

2. Secure all hoistway doors from inside hoistway (permit one door for access, door must be secured without the use of common tools to open)

3. Post signage on each landing doors “Elevator Decommissioned”

4. Engage car top operating device on STOP

5. Remove disconnect fuses, install lockout/tagout

6. De-energize main power and car light disconnects

Hydraulic:

1. Disable pump motor or remove pump motor belt

2. Place shutoff valve CLOSED

3. Car position to be decided by the licensed elevator contractor, contractor to provide a safely parked elevator as needed

Electric/Traction:

1. Car position to be decided by the licensed elevator contractor, contractor to provide a stable parked elevator as needed

2. Remove suspension ropes or members contact relation from drive sheave

Single Bottom Jacks - The following are the options that may be afforded to an Elevator Owner/Operator Variance required

Option1:

The following extra testing would be required for this apply

a. Absolutely no unexplained oil loss

b. Full Load Test annually for a duration of 30 minutes with no more than ½ inch car settling Any failure to comply with these items under this option could result in the requirement of either Option 2, 3 being implemented, depending on the severity. A time frame could be given to comply with these options. This time frame would require a variance request with the Commissioner, Chief Elevator Inspector and/or the Inspector of record Permit required

Option 2:

The installment of a plunger gripper

Option 3:

The installation of a jack with a safety bulkhead It will be the owner/ operator that must make the decision as to which they wish to make.
