New OUSF Funding Request Forms

Pursuant to the Oklahoma Telecommunications Act and Oklahoma Corporation Commission’s Chapter 59 Rules, below are links to the Forms that will be required for Requests for OUSF Funding beginning July 1, 2024. These forms may be used prior to July 1, 2024, at the discretion of the provider and the beneficiary.

Any party objecting to these forms shall file an Application in the Commission’s Court Clerk within 15 calendar days from March 1^st, 2024. If you intend to file an objection, please contact the Public Utility Division at OUSF@occ.ok.gov so that the objection can be scheduled timely.
